A film company faces a citation and the maximum allowable fine after a stuntman for "The Walking Dead" fell to his death a year ago during filming on the hit show's Georgia set, federal authorities said Friday.

According to a report from the Coweta County Sheriff's Office, assistant director Matthew Goodwin told a responding officer that Bernecker meant to fall onto a pad but instead fell head-first onto concrete about 22 feet (7 metres) below. Assistant director Matthew Goodwin claims the accident occurred because Bernecker did not get a "good separation" from the balcony. An on-site medic immediately rushed to his aid, but Bernecker needed to be airlifted to Atlanta Medical Center. He was then placed on a ventilator, but his family chose to turn it off the next day.

The U.S. Department of Labor's Occupational Safety and Health Administration said in a news release that it issued a "serious citation" to Stalwart Films LLC.

Bernecker died July 14, two days after suffering massive head injuries in a fall of more than 20 feet from a balcony to a concrete floor while rehearsing a fight scene with an actor.

OSHA proposed penalties totaling the "maximum allowable" fine of $12,675, for the company's "failure to provide adequate protection from fall hazards".

"This tragedy should serve as a wake-up call for the entertainment industry", said OSHA Atlanta regional administrator Kurt Petermeyer.

Several of the main castmembers of the show took to social media to share condolences soon after Bernecker's death was announced last summer.
